Sciatica vs Piriformis Syndrome: Understanding Your Leg Pain

by | Oct 30, 2026 | Articles | 0 comments

Summary:

  • If you have sciatica, you may feel pain travelling from your lower back into one leg.
  • Piriformis syndrome can cause similar symptoms, particularly around your buttock and leg.
  • Where you feel pain and what triggers it may offer clues, but a medical assessment can give you clearer answers.
  • Your treatment depends on the underlying condition and how severe your symptoms are.
Pain that travels down your leg can make sitting, walking or exercising uncomfortable. Although sciatica is a common explanation, piriformis syndrome can sometimes produce similar symptoms.

If you are considering sciatica treatment in Singapore, understanding the differences can help you know what to expect during an assessment.

What Is Sciatica?

Sciatica commonly occurs when nerve roots in your lower spine become irritated or compressed. Possible causes include a slipped disc or spinal stenosis, where narrowing around the spine places pressure on nerves.

You may experience shooting or burning pain down one leg, sometimes accompanied by tingling, numbness or weakness.

Symptoms may worsen when you sit for long periods or make certain movements. The intensity of pain can also vary, from occasional discomfort to symptoms that interfere with your daily activities.

What Is Piriformis Syndrome?

The piriformis is a small muscle deep in your buttock that helps rotate your hip.

Piriformis syndrome may develop when this muscle or nearby structures irritate the sciatic nerve. You might feel buttock pain extending down your leg, especially after prolonged sitting.

Activities involving repeated hip movements, such as running or climbing stairs, may also aggravate symptoms. However, these triggers are not unique to piriformis syndrome.

Similar symptoms can occur with either condition, so pain location alone cannot confirm the diagnosis.

How Can You Tell Them Apart?

Although symptoms overlap, certain patterns may help your doctor narrow down the possible cause.

Feature Spinal sciatica Piriformis syndrome
Pain location Lower back, buttock and leg Mainly buttock, sometimes extending down the leg
Common triggers Bending, coughing or spinal movements Prolonged sitting or certain hip movements
Other symptoms Tingling, numbness or weakness Buttock tenderness, sometimes tingling

How Is the Cause of Leg Pain Diagnosed?

Your doctor will ask about your symptoms, activities and movements that make your pain better or worse.

Your assessment may include:

  • Nerve checks: Testing leg strength, sensation and reflexes.
  • Movement tests: Checking whether spinal or hip movements reproduce your pain.
  • Imaging: An MRI scan may be recommended if significant nerve compression or another condition is suspected.

Your doctor may also check for tenderness around your buttock and assess how your symptoms respond to specific hip movements. These findings can help determine whether further investigations are necessary.

If your leg pain persists or affects your mobility, our specialists at Spire Orthopaedic Centre can examine your symptoms. Contact us to arrange a consultation.

How Does Treatment Differ?

Many patients begin with non-surgical treatment, although the approach and recovery timeline vary.

Your doctor will consider how long you have experienced symptoms, their impact on your daily routine, and whether you have any signs of nerve dysfunction.

Treatment for Spinal Sciatica

Sciatica pain treatment may include:

  • Medication: Appropriate pain relief or anti-inflammatory medication.
  • Physiotherapy: Exercises to improve mobility and support your spine.
  • Injections: Considered for selected cases of persistent nerve-related pain.
  • Surgery: Considered when significant nerve compression causes persistent symptoms or neurological problems.

Treatment for Piriformis Syndrome

Treatment generally focuses on reducing irritation around the buttock.

Your doctor may recommend activity adjustments, physiotherapy and targeted stretching exercises, where appropriate. Piriformis injections may be considered if symptoms persist.

Surgery is rarely necessary.

When Should You See a Specialist for Leg Pain?

Mild discomfort may improve, but ongoing symptoms can affect your daily routine.

Arrange an assessment if you experience:

  • Pain that persists or keeps returning
  • Difficulty walking, sitting or exercising
  • Persistent tingling, numbness or weakness

Seek emergency care for new bladder or bowel control problems, numbness around the groin or rapidly worsening leg weakness.
